Programs and classes at LDTA are for the full series. We understand that the needs of your family may change, so we will allow withdrawals according to the following guidelines:
All withdrawals must be submitted to LDTA in writing. Withdrawal penalties are determined by the date on which written request is given. A withdrawal is not complete until the written request is received. The account holder will be liable for the full tuition if LDTA is not notified of withdrawal.
This form serves as the written request for a leave of absence or withdrawal from LDT Academy.
WITHDRAWAL POLICY
LDTA’s Programs offer discounted rates based on registration for multiple classes for the full scheduled session. Your tuition will be recalculated to remove the full session discount. The drop-in rate is $20 per class session. If you withdraw from classes, you will be charged the drop-in rate for the classes from the time of registration through the date the form is submitted. Please note, this may result in an amount due. Students who have a balance on their account cannot return to class until the balance is paid.
If the amount already paid is greater than the amount owed, the account holder will receive store credit toward future tuition or purchases or a cash/credit card refund, by request.
If the amount already paid is less than the amount owed, the account holder will receive a final invoice within 30 days of the date this form is received.
LEAVE OF ABSENCE (minimum of three weeks)
For absences of fewer than three weeks, students should work to make-up classes within LDTA's schedule. For temporary withdrawals (leave of absence) of three weeks or more, this form must be completed with supporting documentation. It may take up to one week to approve a leave of absence once this form has been submitted. During a leave of absence:

Students may not attend LDT Academy classes or events.

Payment plans in effect will be paused.

No credits/refunds will be made until student returns to LDTA.
Once the student is able to return to classes, supporting documentation may be required:

Medical Leaves: Letter from doctor asserting the student is ready to resume strenuous activity.

Academic Leaves: Grade report showing passing grades in all classes.

Other: Supporting documentation required on a case by case basis.
To return to LDT Academy, the financial account must be in good standing with no past due balance (other than paused Auto Pay plan).
