Programs and classes at LDTA are for the full series. We understand that the needs of your family may change, so we will allow withdrawals according to the following guidelines:
All withdrawals must be submitted to LDTA in writing. Withdrawal penalties are determined by the date on which written request is given. A withdrawal is not complete until the written request is received and approved. The account holder will be liable for the full tuition if LDTA is not notified of withdrawal.
This form serves as the written request for a leave of absence or withdrawal from LDT Academy.
WITHDRAWAL WITHIN FOUR WEEKS OF FIRST CLASS
If we are notified in writing of a withdrawal within the first four weeks of classes the account holder will be responsible for the amount of tuition due for the number of weeks enrolled at the drop-in rate of $20 per class (first class will be treated as a trial class). Fees will be assessed for the number of weeks enrolled.
WITHDRAWAL AFTER FOUR WEEKS OF FIRST CLASS
If we are notified in writing of a withdrawal after the first four weeks of classes, the account holder will be responsible for 100% of the tuition up to the date of withdrawal and 50% of the tuition for remaining classes.
If the amount already paid is greater than the amount owed, the account holder will receive store credit toward future tuition or purchases.
Cash/credit card refunds will only be given if the student is withdrawing for medical purposes (with a doctor’s written recommendation) or in the case of a family moving out of the area (documentation is required) or in the case of a death or serious illness in the immediate family of the student. All cash/credit card refunds include a $50 withdrawal fee.
LEAVE OF ABSENCE (minimum of three weeks)
For absences of fewer than three weeks, students should work to make-up classes within LDTA's schedule. For temporary withdrawals (leave of absence) of three weeks or more, this form must be completed with supporting documentation. It may take up to one week to approve a leave of absence once this form has been submitted. During a leave of absence:
Students may not attend LDT Academy classes or events.
Payment plans in effect will be paused.
No credits/refunds will be made until student returns to LDTA.
Once the student is able to return to classes, supporting documentation may be required:
Medical Leaves: Letter from doctor asserting the student is ready to resume strenuous activity.
Academic Leaves: Grade report showing passing grades in all classes.
Other: Supporting documentation required on a case by case basis.
To return to LDT Academy, the financial account must be in good standing with no past due balance (other than paused Auto Pay plan).
