Booking Process and Payment Policy:

Upon submission of this intake form, you will be notified of available session times. Your proposed session time will be reserved for 48 hours. Session is booked upon payment or written agreement of payment arrangements. There is no charge for a cancellation or date change request that is made a minimum of 48 hours prior to original session date. Otherwise, a no-show, non-cancellation charge of $500 is incurred. ALL SESSIONS ARE BOOKED ON PHOENIX, ARIZONA USA TIME.
