Thank you for choosing to adopt a pet from Wag-A-Muffins Animal Rescue (WAM)!
Every animal is unique. Every home is unique. We are here to help you find the right match for your household.
 
Before you complete this application please note:

• Most of our animals come to us without any background information so, while we do our very best to evaluate, and will provide all information that we do obtain, we cannot guarantee the temperament of our animals.
• We cannot guarantee the health of our animals. We will share any information that we have, including vet observations and exam notes and all vaccination records; however, some illnesses may not be observed, or the incubation period of a disease means that symptoms could appear after adoption.
• Our screening process is thorough, but quick and efficient. Please apply only when you are ready to actively search for, meet, and bring home a rescue animal.
• A rescued animal will have a period of adjustment when entering your home. This period may be a few days to a few weeks and could result in temporary behavioral changes.
• The adoption fee covers spay/neuter, health exam, up to date core vaccinations*, deworming, and any required treatment for parasites.
• Adopters are financially responsible for allveterinary care and medical bills due or incurred after adoption. This includes, but is not limited to, non-core vaccinations such as bordetella, or any other required medical intervention or treatment.
• If you must rehome your pet, for whatever reason, you must contact WAM first.
• WAM reserves the right to verify all information provided on this adoption application.

*CORE VACCINATIONS

• Core Dog Vaccines: Adenovirus Type 2, Canine Distemper, Parainfluenza, Parvovirus, and Rabies
• Core Cat Vaccines: rabies, feline distemper, feline herpesvirus and calicivirus
   
  Rabies vaccines will only be done on animals over 4 months of age.

We specifically ask this question as rescued animals require time, patience, understanding, and energy! As many of our animals will come from a place of trauma or neglect we may not know the true extent of their needs until the animal feels safe in their home. 

By sending this electronically, I acknowledge that I have completely read and comprehend this questionnaire.

I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any Wag-A-Muffins animal that I am interested in. 

I certify that the above information is correct, and I understand that the information will be verified.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Wag-A-Muffins and its members from any claims, damages, costs, or actions incurred because of the care or actions of the adopted animal.

I accept full responsibility for the animal’s actions at all times, and release Wag-A-Muffins from any liabilities or damages that may be incurred because of adopting such animal(s).

I understand that there is a risk that I may be scratched, bitten, or may come into contact with a diseased animal. I agree to release and hold harmless Wag-A-Muffins from any and all liability for any damage or injury.

I agree to have Wag-A-Muffins complete reference call checks and conduct a home visit inspection to be able to approve my adoption application.

I agree that if I'm unable to keep the animal(s) that I have adopted anymore that I will return the animal(s) to Wag-A-Muffins.
