    The Deed Poll will be drafted and then posted the next working day. One child per application. For example, if you have two children, then you need to submit two applications. Should you have any questions, please do not hesitate to contact us.

  • Parental Responsibility Declaration

  • Everyone with Parental Responsibility must consent to the name change of the child named in this application.

    To process your child's Deed Poll, we need you to confirm that you accept this Parental Responsibility e-Declaration by ticking the box below. 

    By accepting this e-Declaration, you confirm that you have understood the above and that you wish to proceed on the basis that you have either sole parental responsibility for the child named on this application or that you share joint parental responsibility and the other parent/guardian(s) fully consent to the name change. Note this still applies even if the other parent is absent or his/her whereabouts are unknown.

  • Duplicate Copies

  • What is a duplicate, and how many copies do I need to order?

    A duplicate copy is an identical copy of your original Deed Poll. All copies are authentic.

    We recommend at least 7 to 9 copies, as some organisations might keep and not return your Deed Poll. It also allows you to notify several organisations at the same time. Most organisations do not accept photocopies.
