*Acceptance into HEAL contingent upon approval by UCSF Office of Graduate Medical Education.
Please list languages apart from English that you would have some level of comfort using in a clinical setting.
Please provide the contact information below for 3 references. Please do not submit any reference letters. HEAL will reach out to references separately. One of these references must be a residency / program director.
Why do you want to be a HEAL fellow? (Maximum of 500 words)
Tell us about a time when you worked or lived in a community that was not your own. How did you acclimate to this new community and what did you learn? In hindsight, would you do anything differently? (Maximum 300 words)
Please describe a time when you have experienced a failure. How did you recover from it? What did you learn? (Maximum 300 words)
Below are the HEAL Rotating Fellow positions for our 2020 - 2022 cohort. Please read the Role Descriptions on the HEAL website carefully and select the domestic and international placement sites that align with your specialty and areas of interest and where you would seriously consider accepting placement for the 2-year fellowship. You can make multiple selections for both Domestic and International positions.

Please include education (medical, graduate and undergraduate education and other residency/trainings with dates), work, research and volunteer experiences, leadership roles, awards, honors, publications and conferences attended. Additionally, please list any significant global health experiences and with dates and the nature of your involvement.
