If Georgia Academy is unable to obtain this child's immunizations (Form 3231) within 30 days of enrollment, we will notify the primary legal guardian. In order to comply with state law, it is necessary for the parent/guardian to supply Georgia Academy with updated immunization records. Regular updated reports will be required as they are due.
I hereby grant Georgia Academy permission to take whatever action in its judgment may be necessary in supplying emergency medical services to this child. I understand that consistent with the circumstances of the situation and the available time, Georgia Academy will attempt to contact and follow the instructions of the parent, physician, or other person(s) as designated. I hereby grant permission to Georgia Academy to contact and comply with the advice of an available physician, ambulance personnel, or emergency room personnel. I hereby agree that I will be solely responsible for and will promptly pay any expenses which may be incurred by Georgia Academy in making emergency medical treatment available to the above named child.
I authorize Georgia Academy to apply one or more of the following topical ointments/preparations to my child in accordance with the directions on the label of the container, with the exception of insect repellent that can only be applied once daily and only when recommended by public health authorities. It is my responsibility to provide these items when needed.
Georgia Academy agrees to provide care for your child 6:30am-6:30pm, Monday-Friday, January-December.
Your child will participate in Breakfast, Lunch, an Afternoon Snack, and an Evening Snack.
Before any medication is dispensed to your child, a completed Medication Authorization Form must be completed each week and medicine must be in it's original container with your child's name on it.
Your child will not be allowed to enter or leave the facility without being escorted by the parent(s), person authorized by parent (s), or facility personnel.
It is your responsibility to keep your child's records current to reflect any significant changes as they occur, e.g., telephone numbers, work location, emergency contacts, child's physician, child's health status, infant feeding plansand immunization records, etc.
Georgia Academy agrees to keep you informed of any incidents, including illnesses, injuries, adverse reactions to medications, etc., which include your child.
Georgia Academy will obtain written authorization from you before my child participates in routine transportation, field trips, special activities away from the facility, and water-related activities occurring in water that is more than two (2) feet deep.
Your signature below authorizes Georgia Academy to allow two and three year olds to be in the same classroom together when developmentally appropriate.
Georgia Academy updates their website frequently and produces marketing literature. Georgia Academy also submits articles/photographs to local newspapers in an effort to provide community awareness to child care programs and to share our daily activities with the community. Georgia Academy may also display artwork and photos within the community at places such as the library, schools and the Chamber of Commerce. Georgia Academy sends pictures and videos to our clients. Georgia Academy produces promotional video/picture advertisements to be posted on the internet, television, billboards, etc. I hereby grant Georgia Academy the following rights: The right to use my child’s name (first name only), photograph, picture, portrait, likeness, video, and voice (hereinafter collectively known as “image”) in connection with its educational materials or for any other legitimate purpose; The rights to use, reproduce, publish, exhibit, distribute and transmit my child’s image individually or in conjunction with other images or printed matter in media releases and still photography.
I hereby confirm that I have received the Georgia Academy Parent Handbook, and further understand the responsibilities expected of Georgia Academy clients; the policies, procedures, rules, regulations and practices as stated in this document; By signing this document, I acknowledge that I have received, read, and agree with the contents listed in the Georgia Academy Parent Handbook.
In the case of an emergency, Georgia Academy uses Saint Mary's Hospital located at 1230 Baxter Street, Athens, GA 30606.
In the event of an emergency involving my child, and if Georgia Academy cannot get in touch with me, I hereby authorize any needed emergency medical care. I further agree to be fully responsible for all medical expenses incurred during the treatment of my child.
The obligation for full payment of tuition and other fees will continue until the date indicated by the parent/guardian as the date of withdrawal on the proper “Withdrawal Notice” form. The parent/guardian agrees to furnish Georgia Academy with at least two (2) weeks advance written notice of such date of withdrawal. If parent/guardian fails to provide written notice, parent/guardian remains responsible for the full tuition for the two (2) weeks after the child’s last day of attendance plus any late charges or penalties which shall accrue until full payment is received. Children are placed in a specific program at time of enrollment. Request for change must be submitted to the Director. A two week notice may be required to ensure a smooth transition. Request will only be granted if space is available. Changes made in enrollment status may only occur once per year. If any situation occurs during which the child is temporarily withdrawn from Georgia Academy, and regular payment of tuition has been temporarily suspended by the parent, the enrollment will be terminated. Re-enrollment will be based on availability of space and an additional registration fee may be required. Georgia Academy reserves the right to make changes to the fees and conditions of enrollment. Fee changes will be posted at least one month prior to any changes. We have specifically reviewed each of the provisions of this Agreement, and hereby agree to comply with all provisions. We have read and understand the parent handbook and are familiar with its contents. I understand that if I disenroll my child from Georgia Academy, a two week notice is required. If I disenroll without notice, I understand that I am responsible for payment of the following 2 weeks.
Georgia Academy requires every family to have a credit card or bank account on file. Georgia Academy accepts American Express, Discover, VISA, and MasterCard. I understand that I will need to provide this information in person or via phone before or on my child’s first day.
