Georgia Academy agrees to provide care for your child 6:30am-6:30pm, Monday-Friday, January-December.
Your child will participate in Breakfast, Lunch, an Afternoon Snack, and an Evening Snack.
Before any medication is dispensed to your child, a completed Medication Authorization Form must be completed each week and medicine must be in it's original container with your child's name on it.
Your child will not be allowed to enter or leave the facility without being escorted by the parent(s), person authorized by parent (s), or facility personnel.
It is your responsibility to keep your child's records current to reflect any significant changes as they occur, e.g., telephone numbers, work location, emergency contacts, child's physician, child's health status, infant feeding plans
and immunization records, etc.
Georgia Academy agrees to keep you informed of any incidents, including illnesses, injuries, adverse reactions to medications, etc., which include your child.
Georgia Academy will obtain written authorization from you before my child participates in routine transportation, field trips, special activities away from the facility, and water-related activities occurring in water that is more than two (2) feet deep.
Your signature below authorizes Georgia Academy to allow two and three year olds to be in the same classroom together when developmentally appropriate.
