I agree to abide by the following guidelines for managing my prescription(s) for controlled substances. I will only request and receive controlled substances that help in the management of my psychatric care from Psychatric Staff at The Growth Center for Counseling & Wellness.
I agree to inform any other prescibers participating in my treatment of this agreement. If any other prescriber wishes to suggest changes, they can contact The Growth Center during regular business hours, but no changes will be made without such contact.
I will not request or accept the same class of medication from any other physician/prescriber while I am receiving medication from this office. I will disclose all medications to my provider including opioids (such as methadone, suboxone, et al). I understand that if my medicines are lost or stolen, they will not be refilled prior to the next refill date. If I use up my supply of medication before the date of the next refill, I understand that my doctor will not provide extra medication. Medication will not be refilled for personal reasons such as vacation, travel, etc. I further understand that I may suffer symptoms of withdrawal. I will inform my doctor in a timely manner if I miss taking a dose of medication, have an increased need for the medication, or have difficulty taking the medication as prescribed. If I find the current dose of medication is no longer adequate.
I agree to use only one pharmacy for filling all my controlled substances. Any changes to the pharmacy of record MUST be coordinated with your provider or you will be out of compliance with this contract resulting in the termination of your psychiatric treatment. I will complete an in-office ("face to face") appointment a minimum of every ninety (90) days with my provider.
While this contract is in effect, I will not abuse alcohol or other illicit drugs. Combining medications with alcohol or certain other medications, such as benzodiazepines (alprazolam, diazepam, and clonazepam, et al), increases the risk of experiencing serious or life-threatening side effects resulting in permanent impairement or death. Other prescriptions or over-the-counter medications may significantly increase the risk of side effects, including overdose. Always read the warning labels on any prescription or over-the-counter medications before taking and take only as directed. Discontinuing certain medications, such as an opioid or benzidiazepine, may lead to withdrawal symptoms, which may include: leg or abdominal cramps, muscular pain and tremors, piloerection (goosebumps), nausea, vomiting, appetite changes, heart palpitations, panic attacks, mood swings, diarrhea, dilated pupils, thinking and memory difficulties, and sleep disturbances. Symptoms will vary depending on the controlled substance medication you are taking. As a part of treatment, drug screening may be required.
I will not sell or share any opioid, benzodiazepine or other controlled substance medications. If I violate the terms of this contract, I understand that my prescriber at The Growth Center will no longer prescribe controlled substance medications for me. If this occurs, I understand that I may receive care elsewhere or continue with my current provider and not receive controlled substances. If I change primary care providers, pain doctors or other provider associate with these medications, I agree to allow my current provider to contact my new provider to transfer medical information including information about my psychiatric treatment.
