You must complete your 90-day probationary period to be eligible for the Non-FMLA leave.

Leaves may not exceed six (6) weeks within a 12 month period.

A Non-FMLA leave of absence (LOA) may be given if you are not eligible for a FMLA leave because: 1) You have not met the FMLA time or hours requirements or 2) Your reason for taking leave is not a reason which is protected by FMLA

If you are approved for a Non-FMLA leave, the Company will make reasonable efforts to return you to the same or similiar job you held prior to your LOA, subject to staffing and business requirements. Should an employee not return after the approved leave of absence, the employee will be considered as having voluntarily resigned his/her position.

Engaging in other employment during a medical leave or personal leave is prohibited and will, in the discretion of the Company, result in disciplinary action up to and including discharge.
YOUR PAY & BENEFITS WHILE ON LEAVE

During your LOA, you will not receive your regular pay.
- If you are participating in the Driver Sign On Bonus and/or Referral Bonus Programs, you will not receive your remaining referral bonus payments while you are not actively working and out on LOA. Your bonus payments will resume once you are in active status with the Company.

You will be required to use vacation or unpaid time off, if available.

You will need to continue to make your share of the premium payments on your health insurance to maintain health benefits while you are on leave. You have a minimum 30-day grace period in which to make premium payments. You will be notified of the amounts of these payments and how to send payment during your leave.
RETURNING TO WORK
If your circumstances change, you need to notify our Human Resources office immediately.
