For your event, please submit the following:
- A Certificate of General Liability Insurance in the amount of $1,000,000 naming the Summit Recreation and Social Club, Inc. as “additional insured” and the certificate holder for the date(s) of your event.
- The Certificate of General Liability Insurance must contain an Endorsement including coverage for property damage while the Club property is occupied by the permittee, for the event that reflects that the Summit Recreation and Social Club, Inc. is an additional insured for the event. Coverage for property, with property address must be explicitly stated.
- **If you intend to have alcohol (either provided by you, or guests bringing their own) you must have an alcohol endorsement on your insurance policy for day of event.**
- Signed Hold Harmless agreement (Provided by Summit Recreation and Social Club after General Liability Insurance has been submitted).
- Without the above documentation, Summit Recreation and Social Club reserves the right to deny your guests attendance.
