Parent Information
Our program offers a volunteer experience to youth ages 14-18, in a variety of areas within our hospital campus.
Junior volunteers must commit 2 hours of service per week.
Uniforms will be made available to accepted candidates.
Acceptance will be determined after the interview process, PLEASE SHARE WITH YOUR CHILD THE IMPORTANCE OF A FIRST IMPRESSION.
In order for your child to become a Reid Junior Volunteer, we need your consent and your involvement in helping them have a productive experience.
Please complete, sign and return the bottom portion with your child’s application packet.

Parent/Guardian Consent
I understand that my child is applying for the Junior Volunteer Program at Reid Health and I give permission for him/her to serve in that capacity. I understand that my child will be provided with the orientation and training necessary for the safe and responsible performance of his/her duties. He/she will be expected to meet all the requirements of the position, including regular attendance and adherence to hospital policies and procedures.
