This written contract is a document stipulates that Dominique Rochelle Beauty (Salon Rochelle, LLC) will provide on-site Hair and/or Makeup service(s) for bride and her guest(s) listed below, along with policies and terms to be agreed upon to follow for both parties.

Contract Policy:

Contract will be signed by the person booking services for a particular event, and will specify the location, date, times reserved, and number of people receiving services. When contract is signed,  retainer payment must be paid at the time of contract signing to guarantee each reservation will be accommodated. Retainers payments of 50% total balance must be paid to hold any date. Any amount less than $600 is required to be paid in full. This contract with full policies and terms to agree to will be emailed to the client and returned to Dominique Rochelle Beauty (Salon Rochelle, LLC) with payment(s). Only person signing contract is responsible for all agreements and collections of fees due to services provider. Contract will become void after 72 hours/ 3 days from the time it is emailed for initial booking. If your contract and payment within that time period is not received then your date may be forfeited to another booking. New contract will be requested at the discretion and approval from service provider.

Appointment/Reservation Policy:

Due to busy schedules and working by appointment /reservation only, partial/full payment is required up front. If service booking is considered last minute which is less than 90 days full payments are required. Partial payments will only be deducted from full balance reservation pricing when booking to hold your date in advance. To secure any appointment/reservation for your special event, you must pay partial/full agreed upon amount. Please be advised, dates and scheduled service appointments/reservations will only be reserved when payment is made and contract is signed. New contract will be made or edited if service needs are changed. Your payment to hold the date will be non-refundable and non-transferable if you cancel your appointment for any reason. Remaining Balance:

Remaining balance payment agreement is expected on selected date(s) payable to service provider 14 days before reserved event. If the client has any balance, service(s) will NOT be rendered if not paid. If no communication is made for arrangements service provider will cancel due to negligence. If previous payments were made to hold date(s) for retainer they are non-refundable and non-transferable. Please review cancellation policy.

Cancellation Policy:

You may cancel your services at any time however; you will forfeit all retainers and/or payments made towards balance. Cancellation of this contract and services must be canceled 30 days before event or full balance is required to be paid in full. In the event, that an makeup artist/stylist of Dominique Rochelle Beauty (Salon Rochelle, LCC) is unable to perform services for any reason including but not limited to sickness, hospitalization,  accident, weather, unforeseen acts of god etc., reasonable accommodations shall be made up to provide another Service Provider or a full refund of retainer and payments made for services not provided in allotted time frame. 

Right of Refusal Policy: In event that conditions to perform services are unsafe or at risk for any of the our contracted makeup artists/ stylists of Dominique Rochelle Beauty (Salon Rochelle, LCC), we reserve the right to leave, refuse, and cancel any client services for any reason including but not limited to sickness of any kind that are communicable diseases (like conjunctivitis or other eye infections, influenzas, common colds/fever, norovirus, coronavirus, meningitis, HFMD,  MRSA, etc.),  verbal abuse, and, physical abuse. If notified in advance about medical conditions that are communicably contagious please make sure advance measures are taken care of like having a doctor's note for documentation stating that you are cleared and safe at no risk to proceed with services as well as a doctor calling to confirm written documentation. We also require that pets be secured safely away in a separate room. Please note if services are refused, canceled, and we leave due to these circumstances, retainers and balance payments made towards services will not be refunded or transferable. 

Travel/Hotel Policy:

All travel, hotel, and misc. expenses must be covered by guests booking services for service provider.
•Travel starts at $50 per hour for driving (roundtrip). Additional traveling fees like airfare, rentals, lodging, day rates etc. will be discussed in further detail and added to contract for accommodations of destinations if applicable. If any tolls and/or parking, it will be discussed on day of if applicable for payment and noted on contract.

• A hotel will be needed for locations more than 1.5 hours, start times are earlier than 5am or finish later than 6pm. The hotel location and fee will be discussed and added to the contract.

 

Additional Contract Policies and Fees:

All additional fees must be paid if applicable.

For services more than 4 or depending on what is needed, assistance will be brought in for an additional $150 flat fee per artist. Artists may also be brought in if you have a limited time to get ready or at the recommendation of Dominique Rochelle Beauty (Salon Rochelle, LLC). It can also be requested by you.

Note: Artists do full services for guests and are highly trained professional that we hand pick to be the best at what they do.

- A late fee of $50 after 30 minutes will be charged with $1 per minute following thereafter

- If you would like Dominique Rochelle Beauty (Salon Rochelle, LLC) to stay after the initial application it will increase at a rate of $75/hr. for Lead Artists and $50/hr for each Makeup artist/Hair stylist staying on the team. 

***Day of Services added will be charged at additional fees by services provider if there is time to accommodate for last minute or emergency services added to invoice. All services and services added must be paid only by contracted client to service provider. All payments must be made prior to any services that are rendered.

***Artist hired outside of Dominique Rochelle Beauty (Salon Rochelle, LLC), it is required that we must be informed of other artists hired outside of our business establishment. We understand if makeup and/or hair services are not all provided by our business but we are not responsible for anyone or anything outside of our contract agreement. If such issues come from another business like being late, no shows, medical issues, improper sanitation, unsatisfied services provided, using our tools/products or anything else that may cause issues or conflict to provide our services properly we have a right to leave or charge any fee amount to cover what is needed. Our business is liable only for what is covered by our insurance coverage to protect us and those we serve in our agreement. Please review our Kit Policy.

Kit Policy:

All tools and products are to be used and touched only by Dominique Rochelle Beauty (Salon Rochelle, LLC) and her beauty team. No other businesses or guest(s) should be allowed to touch any belongings of our business for any use. No pets, children, drinks, or smoking should be allowed next to the area of our tools or products. This is in place to provide and have the best quality services for our guests by making sure our tools work and everything is correctly sanitized and to eliminate allergic reactions for each guest as well as having what we need. If anything is touched or broken no matter the value there will be a fee at the current value charged to contracted client. 

Location Policy:

It is advised to have hair and/or makeup set up in one location. We require all guests to visit that same location. If we need to move to other locations or need to be set up in more than one location there will be a fee valued at $100 or more per location at the discretion of the service provider. As well as, if a location we do not own requires a fee be paid for services done for you from us the contracted client must cover all fee payments from that location.

Use of Image(s) and Videos(s):

With permission, Dominique Rochelle Beauty (Salon Rochelle, LLC) would like to use/share photographs, video, audio, and any other digitally or chemically stored media that is captured or recorded by Dominique Rochelle Beauty (Salon Rochelle, LLC) of this event on website, social media, brochures, flyers or other advertising if given permission. If so Dominique Rochelle Beauty (Salon Rochelle, LLC) will always give credit(s) to those who participated and possibly refer for other business opportunities on our vendor preferred referral list. Guest(s) agreeing to release use of his/her image or video for such purposes also agree to give name(s) and contact information of any professional(s) involved for crediting reasons and networking opportunities for future business. Connections with photographers and videographers as they have rights to their work own works well is especially important even if you decide to share photos or video with us from them and credit us for our work. Connections will be made and additional permissions asked of other vendors from event. 

 

 

 

This contract constitutes the entire agreement between the Client and Servive Provider (and beauty team). Supersedes any prior understanding or representation of any kind preceding the date of this agreement. There are no other promises, conditions, understandings or other agreements, whether oral or written, relating to the subject matter of this Agreement.

 

 

By signing this contract, I (the client), have read and understand all the terms and conditions outlined above as well as daily planned schedules, locations, and hair and/or makeup services listed on a separate page from this contract. I understand that I will be financially responsible for submitting all payment(s) due as well as additional fees if applicable to Dominique Rochelle Beauty. I will abide by this contract. This contract is binding.

 

 

By signing this contract, I Dominique Rochelle Beauty, have read and understand all the terms and conditions outlined above. I understand that I am contractually agreeing to be present on the date outlined in this contract and will show up prepared to service all guests of the client I am contracted by. I will abide by this contract. This contract is binding.
