Submission of this adoption application does NOT confirm acceptance or guaranteed adoption of the pet. Please note, we qualify all applications and choose from the most qualified and best suited for the pet. If your application is accepted - an in home visit will be scheduled. Every member of the household will need to be present and any other pets that reside at the property. We need to assess the adoptee’s behavior and compatibility subject to our discretion - A HOME VISIT DOES NOT GUARANTEE ADOPTION.

Placement of the pet in the FOREVER HOME will have a 7 DAY NO QUESTIONS ASKED HOME TRIAL PERIOD. In the event the placement is not acceptable - the pet will be returned to us and any monies for the placement will be refunded. After the 7 DAY Home Trial period, no refunds will be made unless agreed upon by both parties.

This pet will not be transferred, gifted, re-homed or put up for adoption, surrendered at any shelter or rescue without notifying us prior to any action taken at any time thru the life time of the pet. We are not responsible for the temperment of the pet. Training classes with pet behavioural professionals is highly recommended for all pets.

The pet has received veterinarian medical care for parasites, malnutrition, and other conditions associated with abused or abandoned animals. The pet has received age appropriate vaccinations and will require additional ANNUAL vaccines, antibiotics flea/tick and heartworm monthly medications in accordance with your veterinarian’s recommendations. Spay/neuter is mandatory and will be provided at Fix A Friend prior to the first female heat or no later than 6 months.

We will provide pet history, medical records, and known facts and cirumstances and makes no guarantee or warranty for physical, tempermental or medical conditions that may occur during the lifetime of the pet. Pet is adopted "as is/where is" with as much information as possible. No representation of pet's lineage, parents breeds is guaranteed. Only a DNA test can best produce the actual mix of the pet,

Adopter understands that pet's health costs, flight fee, age appropriate vaccinations food, and Health Certificate has been covered by us. To be able to continue our RESCUE * FOSTER * TRANSPORT * ADOPT Program these expenses are the "Adoption Fee" that we charge for the pet. We are an ALL VOLUNTEER organization where 100% of funds received go to rescue efforts. No one receives any form of compensation. The Adoption Fee is $350 and includes the cost of 2- vaccinations, a crate,starter food, a leash, collar and spay/neuter. Adoption fee is non-refundable after 7 day trial period. Payment is accepted by CASH, CHECK and VENMO at time of placement.

All the information I have provided is true and complete, The pet will reside in my home and I will provide quality dog food, fresh water, training, indoor shelter, carefully monitored outdoor exposure, and plenty of exercise and affection. Neither I, or anyone in my household has been charged with or convicted of animal cruelty or abandonment. The pet will receive regular visits to a licensed veterinarian including a visit within 2 weeks of adoption to stay current on any and all vaccinations and for examination, a fecal analysis and treatment for any parasites and conditions. I agree to adhere to ANNUAL age appropriate veterinarian visits and administer monthly flea/tick and heartworm preventative 12 months of the year. I understand this is NOT NEGOTIABLE
