CONSENT TO APPLICATION OF SEMI-PERMANENT MAKEUP PROCEDURE
I am over the age of 18, not under the influence of drugs or alcohol, and desire to receive the indicated semi-permanent cosmetic procedure. The general nature of cosmetic tattooing as well as the specific procedure to be performed has been explained to me.
Bookings will be required to pay for a downpayment with a minimum amount of $100 per person and per appointment.
Cancellation of Appointment 72 hours timeframe before the appointment but subject to cancellation fee, non-refundable and non-transferable downpayment. If the cancellation has been made within the 72 Hours window. Please refer to ACCC (Cancelling a Service Law). Every customer needs to fill out the form for record-keeping and group sessions are still considered but every member has to fill out the agreement.
WHAT'S THE GENERAL PROCESS LIKE?
It is a two-step treatment, consisting of an initial procedure and then a touch-up between 6 to 8 weeks after. Additional treatments cannot be undertaken until the area has completely healed. After taking your before photo, I will draw the design in pencil so you will get an idea of the shape and form. (Please note that nobody’s face is totally symmetrical and you need to be realistic with your expectations. There is no point in trying to make them identical or match a photo of your favorite celebrities’ if your face shape looks nothing like theirs.)
Once we have agreed on a final shape (ultimately it is your decision—but I will always voice my opinion on what I think suits you aesthetically),. Shortly after that, a topical anesthetic will be administered to ensure comfort. Once the area for treatment is numbed, I will shape it according to the symmetrical measurement and mapping, and then we will decide on the color in which pigment will be used. During healing, it’s normal to experience some pigment loss; this is why we have the touch-up in place.
WHY DO I NEED A TOUCH-UP IF THIS IS SEMI-PERMANENT MAKEUP?
Because color is implanted in the facial area there tends to be more fluid circulation than other parts of the body. Your face sees the most wear of any piece of you—exposure to the sun, food, facial products—the list goes on and on. All of these things cause the permanent color to fade; this is why you need a touch-up.
Note: Touch Up Fee is required based on how long you had your last session.
AFTER YOUR TREATMENT:
- You are advised to keep the skin clean and dry—this means the following is advised.
- No swimming, heavy sweating for the first 10 days or otherwise getting the area wet.
- Avoid direct sun exposure or tanning for 3-4 weeks after the procedure.
- No facials, botox, chemical treatments, and microdermabrasion for 4 weeks.
- Avoid sleeping on your face for the first 10 days. NO PICKING, PEELING, SCRATCHING, or RUBBING THE TATTOO, this may result in a loss of color and scarring.
- NO CLEANSERS, CREAMS, MAKEUP, OR ANY PRODUCT on the treated area for 15 DAYS.
- Within 2 hours after the procedure clean/pat the area with a cotton pad with clean water and allow it to air dry for 2 to 3 minutes and put the advised ointment for a faster healing process. Let any scabbing or dry skin naturally exfoliate off.
PLEASE NOTE: TREATED AREA WILL APPEAR DARKER AND BOLDER DUE TO NATURAL REACTION AND HEALING FOR THE FIRST 10 DAYS. THIS IS VERY COMMON FOR ALL SEMI-PERMANENT COSMETIC PROCEDURES.
CAN YOU GUARANTEE YOUR WORK?
Unfortunately, I can’t. How the skin will react is different for everyone and many factors are to be taken into consideration. Skin type, age, scarring, diet, stress levels, facial products used, etc. all play a key role in how the pigment will hold, which is why it is important to know that more than one touch-up may be required. I can guarantee that your face is important to me and that I will act in accordance with my experience and expertise to give you my absolute best. I can guarantee that I will strive to make you gratified and content every time. But there are no guarantees in this industry.
I have reviewed and received pre and post-procedure instructions and I will strictly be there for such instruction. I understand that my face may not be symmetrical, therefore, one eyebrow(for example) may be higher than the other in the procedure. If I am on any medications for depression or any other mood-altering prescription, I will advise my technician. If I have ever had cold sores, I will consult with and strictly follow my doctor's instructions before contemplating any semi-permanent cosmetic procedure. Also, any form of a reaction with cosmetic pigments, anesthetic, or needling always varies from person to person and the technician will not be liable or involve in any related that will be beyond the capacity.
By signing this, I agree to the procedures discussed. I understand that taking photographs before and after is required. I certify I have and initiated the above paragraphs and have full explanations of my understanding and consent to this procedure. There is no 100% guarantee ink will hold on to a camouflaging or correcting. I accept full responsibility for the decision to have this cosmetic tattoo done and understand I have a 100% satisfaction guarantee on procedures performed only (NON- REFUNDABLE).
