  • Please Note:

    For a standard 3 hour event, the recommended offering is $20 per child and $25 per family. Encourage non-parents to contribute towards this ministry.

  • In order to provide excellent care, please give your best estimate of the expected numbers of children below. At least one week prior to the event, please update this form. Check the bottom of your confirmation email for a link that says "edit this submission."

  • Childcare will be open for drop off 10-15 min prior to event start time. Until that time, parents need remain with their children.   Children must be singed in on the attendance sheet with a security number.

    We are not a drop-off extended daycare but childcare for the event only, parents need to stay on site at all times.   

    Parents need to pick up their children in the specified time with the security number sticker.  No one, including other volunteers or staff may sign out a parent’s child from the classroom unless they have the security sticker and are on the emergency back-up list of authorized individuals that can sign out.

    Children ages 0-12 months parents must fill out a yellow Special Instructions form.  

     

    Bottles/drinks must be labeled with name of the child.

     

    We are a PEANUT FREE ZONE- Please do not send peanut products with your child

     

    Snacks provided will be listed on drop off.   Fishy Crackers, cheerios, animal crackers and water or juice (reduced sugar) will be provided.
