The Health and Safety Audit requires both a signature from the Primary Faculty Course Leader and a signature from the appropriate designated college Dean. The process for signature is as follows:
- Faculty will electronically sign and submit below
- Once submitted, a report will automatically be generated with the above information and emailed to cge@simmons.edu
- The CGE staff will email the completed Full Proposal in a document form to faculty
- Faculty will have the approprate Dean sign off on the Full Proposal.
Please return the signed document to the CGE by June 15.
