  • Parent/Guardian, Billing and Emergency Contact Info

  • NOTE: For purposes of this form, the Primary Parent/Guardian is the first person to be contacted in case of emergency and/or any TRYPO-related business. The Primary Parent/Guardian is also the main contact for TRYPO's weekly e-mails and text messages (TRYPO text messages typically include important updates about traffic, last minute cancellations or schedule/room changes).

  • [If there is no secondary parent/guardian, please skip to billing contact.]

  • BILLING CONTACT

For purposes of this form, the billing contact is the person responsible for handling tuition invoices and making sure they are paid in a timely manner. TRYPO invoices will be distributed via e-mail, so it is important that you enter at least one (1) valid e-mail address. If you would like to enter multiple e-mail addresses, please separate them with a coma and no spaces (i.e.: max@example.com,babou@example.com,etc@example.com).

  • EMERGENCY CONTACT

In the event that we cannot reach the primary or secondary guardian in the case of an emergency, event, or transportation issue throughout the season, please provide us with one additional emergency contact as a back-up.

  • Communication Preferences

Staying informed is tremendously important and we do our best to provide information to TRYPO families. The majority of our commuication takes place via e-mail. TRYPO typically sends one e-mail a week with reminders, member information, and opportunities. There could be one or two snail-mailings per season. Please tell us how best to communicate with your family about your TRYPO membership.

  • School/Private Teacher Info

  • TRYPO recognizes the value and importance of school music programs in their communities. TRYPO highly recommends and strongly encourages its members to be active participants in those programs.  TRYPO and the schools share a common goal of educating student musicians.

  • EMERGENCY MEDICAL TREATMENT

In an emergency situation, every effort will be made to contact the primary parent/guardian and/or emergency contacts as designated in this form. It is possible that a situation might arise which requires that your child receive immediate medical care at the nearest medical facility. However, in the final disposition of an emergency case if parents/guardians or emergency contacts cannot be reached, the judgment of TRYPO's staff and representatives will prevail in regards to securing the safety of its members. If at any time any of the above information changes, I (the Parent/Guardian) will notify TRYPO's executive director in writing. Please check below if your child may undergo routine and emergency medical treatment.

  • Volunteer and Fundraising Policy

TRYPO Member tuition accounts for only 42% of TRYPO’s total income, and TRYPO works very hard to make up the difference! We need your help in various activities throughout the year! TRYPO asks all parents with musicians in a large ensemble (YPO, SYM, WS, PE) to assist us as a volunteer three times throughout the season. Volunteer opportunities range from helping out at your child’s rehearsal, concert or other special event to working on special projects with us in the office or in your free time. (What types of activities? It could mean helping TRYPO thank a foundation or donor by attending an important event with us or writing thank you letters. It could mean helping TRYPO raise funds with a raffle ticket sale with donated prizes, such as a night on the town at the Pittsburgh Symphony or attending a Steelers game.)

We also understand that your time is precious and if you would prefer to opt out, we ask for a one-time tax-deductible contribution of at least $150 before December 31, 2019.

  • The TRYPO Member and Parent(s)/Guardian(s) agree that the electronic signatures on the last page of this form are intended to authenticate this writing and to have the same force and effect as a manual signature.

  • What are your goals as a member of a TRYPO program?

  • The TRYPO Administrative and Artistic Staff are very interested and dedicated in the growth of each member in our large ensembles. We would like to have feedback from you as to what your goals are this season.
