The Private Lesson Instructor has agreed to provide an individual Lesson Plan to RASOF for each student, to track the Student's attendance and progress, and to provide a Lesson Report to RASOF at the end of the calendar year and at the end of the school year. The MYS Administrative Liaison will ensure the paper work is turned into the ROSOF in a timely manner at the end of the calendar year and the end of the school year. The Instructor is accountable directly to the Student and Parent/Guardian (if Student is a minor). RASOF does not employ or supervise Instructors. The Instructor may contract separately with students or parents/guardians to set clear expectations regarding progress and attendance.*

With the help of the MYS Artistic Liaison, the Private Lesson Instructor and Applicant may determine the time, schedule, location and manner of providing lessons, such as via Zoom or other video conferencing platform. The Instructor will confirm the arrangement in a Lesson Plan provided to the Applicant and to RASOF. The Private Lesson Instructor and Applicant agree to follow applicable State laws and guidelines for safety during the COVID-19 pandemic.*

The Student agrees to attend all scheduled lessons, to prepare for them by practicing as recommended by the Instructor, and comply with all rules and behavioral expectations of their school orchestra program.*

The cost of each lesson is set by the Private Teacher, agreed to by RASOF and managed by the MYS Artistic Liaison. The Applicant agrees to pay Instructor $5 per scheduled lesson and to comply with the Instructor's attendance and notice requirements for absences. The Applicant acknowledges that payment may be due for missed lessons and that failure to pay may result in termination of lessons under this program.*

PRIVACY ACKNOWLEDGEMENT AND WAIVER OF LIABILITY
RASOF and MYS agree that the Applicant's personal information will not be shared except among the Instructor(s) and parties to this agreement in furtherance of the Student's music education, except with the Applicant's express permission. The Applicant acknowledges that the private music lessons arranged under this agreement are independent of Roseville Public Schools. The primary relationship and coordination of lessons is by agreement between the Applicant and the Instructor, and the Student's parent/guardian is responsible for supervision of lessons. RASOF and MYS provide only funding and coordination assistance to facilitate private music lessons arranged by the Applicant and the Instructor. The Applicant agrees to hold RASOF and MYS harmless, assumes the risks of accidents or injuries in connection with the private music lessons under this agreement, and waives liability for claims against RASOF and MYS, their agents, employees, contractors, directors, and volunteers, up to and including ordinary negligence, excluding only gross negligence, recklessness or intentional acts.*
