I authorize Polished Business to regularly charge my card on the amount indicated in this form on a monthly basis, and shall remain in effect until I request cancellation or transferance. I understand transfer of subscription requires 3 days notice and cancellation of subscription requires 30 days notice.
I likewise certify that I am the authorized user of the Credit Card that shall be submitted through this form. If my card details should change, I will notify Polished Business at least 7 days prior to the next billable date. I understand that should a charge bounce, I will incur a $10.00 dishonour fee and my card will be attempted to be processed again. Should payment be unsuccessful again I may have interrupted access to my software subscription.
