Before you complete the membership form please read all of the information below and make sure you are aware of all the club rules and what we expect from you as a parent and a cheerleader
1. I understand that when I accept my place on a team that I am fully committed to SS Stars for the whole of 2022/23 season and will attend ALL training sessions
2. I will advise coaches of any absences from training as soon as possible, this includes any illness, holidays, school events as soon as possible. A cold or party is NOT an excuse to miss training especially 3 weeks prior to a competition when sessions are compulsory (please advise any dates you have already booked at the end of this form)
3. I will arrive on time to all training sessions, wearing the correct clothing to take part. (As we train in a public sport centre please make sure you have a crop top or leotard not just a t-shirt)
4. I will remove all jewellery and ensure my hair is up and tidy. any recent ear piercing must be covered with tape during the 6 weeks healing period but will have to be removed for competitions
5. I will put every effort into all training sessions and always try my best and work as a team. I will also practise/condition at home. if I do not maintain by skills to enable me to improve I may be asked to step down from my position on the team
6. I understand that if I miss a session I will be letting my whole team down not just myself and absence could lead to me being removed from the team
7. I agree that the Coaches decision is final and understand that any decisions will be for the good of the team
8. I will treat all members of the team with respect and any bullying or disrespecting any team member or parent could lead to being removed from the club. This also includes anyone who is found to be spreading gossip or rumours about the club, or coaches. This includes posting anything on social media which will bring disrepute to SS Stars (If I have a problem with someone in the club please come and speak to the coaches directly)
9. I agree to pay all fees required for season 2022/23 by SS Stars for the whole season, this includes training fees, competition fees, membership, uniforms, spectator tickets. Should I leave part way through the season, I understand that I will be responsible to pay any outstanding fees already paid by the club for entries and tickets, these MUST all be paid by the required date as issued by the club or an additional late fee will be added to any outstanding fees
10. I understand that I am responsible for getting to and from competitions, and if I am not able to attend I must arrange for someone to take my child, however if you are still not able to transport your child to competitions you must inform the coaches as soon as possible. If we have to remove you from the team and entry fees have already been paid and tickets ordered, I understand that I are still be liable for these costs
11. I understand that all training sessions must be attended even if I am ill (unless covid related) 3 weeks prior to any competition training is compulsory, if I miss a session it could lead to me being removed from the team. This includes any additional sessions the week of the competition to ensure the whole team can practise full out routines ready to compete. If I am removed from the team due to non attendance for no valid reason other than COVID I am responsible for all of my competition costs and NO REFUNDS will be issued
12. I understand that a uniform and cheer shoes must be purchased and agree to pay all fees for this and understand that we are responsible for checking it fits and is kept in a good condition. Should we need a new uniform or if it needs to be repaired I must inform the club at least 8 weeks prior to a competition to enable the club to order a new one. If I need to order a new one I must pay 50% deposit prior to ordering and pay the full balance before I am given my uniform (the club may purchase your old uniform one back if we need it for another team/member however we will only accept this if it is in excellent condition so the club can resell it on)
13. I will ensure my child is picked up and dropped off at the correct time, I understand I must wait for my child to be taken into the hall by a coach before I leave and must be collected on time. No one will be allowed to leave with out parent or guardian collecting.
14. If I need to speak to a coach I will contact them before the session to arrange a suitable time to speak to them
15. I will only contact Coaches via the clubs contact methods, I will not contact them direct on their personal social media account or mobile. If coaches do received messages this way you will not receive an answer to anything sent to their personal accounts unless they have contacted you this way for anything related to SS Stars16. At competition everyone is expected to cheer on the each team that is competing in the same session as you, and you are expected to stay for the awards and remain in you full competition uniform
17. I agree to being photographed or filmed by a member of SS Stars for advertisement or for training purposes
18. In the case of an accident, I agree for any necessary treatment to be administered in the absence of a parent/carer. I will advise of anything which I am allergic too or any medical conditions, so we can have a note of these on file
We will contact all parents/carers via our Facebook Page (messenger), Whats App or via Email. If you are on Facebook please make sure you ask to join our parents Facebook page, so you can keep up to date with all information.

All importance information regarding competitions, fees and training dates will be emailed to everyone, any last minute information regarding sessions cancelled etc will also be posted on our facebook page, parents page or whats app.
