All the information given on this application is true and correct to the best of my knowledge. I read and accept the Best Academy After-School Enrichment Program handbook. Best Academy has my permission to use my child’s photograph publicly to promote the institution. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
Best Academy must be notified ten school days in advance of the date a child is to be withdrawn from annual or semester enrollment; failure to do so may result in additional tuition being charged. For withdrawal from annual or semester enrollment, a partial refund will be given after the tuition has been recalculated for weekly enrollment and a service fee of $100 will be charged. No refund or credit will be given for days missed due to natural disasters, including power outage of the building, and for personal reasons such as trips or sicknesses. No refund will be given for days enrolled on a daily or weekly basis.
Half-Day/ Full-Day Policy
For Half-Days and Full-Days, all students must register ONE week in advance to enroll. A minimum of four students must be enrolled in the Full-Day or Half-Day program in order for the program to be offered. For the Full-Day program, parents must send a lunch and drink with their child and Best Academy will offer a morning and afternoon snack.
