Completing this form will provide us with all the information we we will need to prepare your rental instrument, accessories and book order.

If possible, bring your child with you to pickup the instrument, they can see a real music store and see our students having fun learning to play.

All instruments are approved by music educators both locally and nationally. If for any reason or at any time your instrument does not meet expectations, we will make it right.

We keep a list of the required books and supplies for most local school districts. Some individual teachers may have unique requirements. Normally they will provide you a list.

If you received a list from your teacher, school, or district, please itemize the required items in the last question below, so we can provide you with the specific requirments.

If you don't have a list, no worries, we do.

If you are unsure of what you need, or if you change your mind, no worries, we can take care of any changes when you pick up your instrument, or call or chat with us.
