  • This form does NOT register your child for Confirmation or First Holy Eucharist. Please complete separate forms to complete these sacraments.

    Family Faith Formation is centered on forming the entire family in faith, not just the child. Our eight Family Formation Nights are designed for parents and children to learn side by side, giving you a shared foundation to talk about, pray with, and live out at home. We strongly encourage at least one parent to attend each night, as your presence is essential in helping your family grow as a domestic Church. Please take a moment to review the calendar for all dates and times. Our first gathering will be a welcoming night, where we will outline the vision and guidelines for the year ahead.

  • Flocknote, an email, and a texting app will be used for all Faith Formation communication and updates. Registration for Flocknote is a mandatory part of Faith Formation. Simply text ‘SJOTL’ to 84576 to sign up. You may also register online at https://stjudeofthelake.flocknote.com/

  • Financial Assistance

    Catechetical ministry is a foundational ministry of the Church. Therefore, it is important that all children have access to this important component in their formation as children of God.

    We also believe that parent involvement is necessary. We have a wide variety of volunteer opportunities. Partial financial assistance is available for Family Faith Formation with volunteer hours. Please contact Deacon Frank Pavlick at

    Fee Waiver for Catechists

    Those parents who are called to the ministry of catechism with Family Faith Formation may have the fee waived for one of their children. Please contact Deacon Frank Pavlick at 651-426-3245 ext. 222 or fpavlick@stjudeofthelake.org if you wish to be a catechist.

    Attendance

    Family Faith Formation is part of a lifelong journey. To receive all the blessings and benefits, class attendance is expected. Disruptive behavior affects all those in the classroom, it makes the catechist’s job more difficult, is detrimental to the formation of other young people in the class, and does not reflect Christian values. To minimize disruptive behavior, the attached Code of Conduct will be followed in every Faith Formation class.

    Photo Release

    I, the undersigned, do hereby grant permission to St. Jude of the Lake Catholic Church to use the image of my child(ren). Such use includes the display, distribution, publication, transmission, or other use of photographs, images, and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the St. Jude of the Lake website.

  • CODE OF CONDUCT

    We believe in the dignity of every person. That dignity must be respected as we gather for weekly faith formation. Giving proper respect to others includes:

    • Respectful speech. Do not talk when others are talking, either catechists or other students. Name calling or other verbal abuse will not be tolerated.
    • Attentiveness. Follow instructions given by catechists or other leaders. This will help to make faith formation go smoothly and help everyone to learn.
    • Respect of other’s personal space. Keep your hands to yourself. There is to be no hitting, pushing or any conduct that is aggressive or may cause harm to others.
    • Appropriate language. Using God’s name disrespectfully or frivolously will not be tolerated. It is important for all children to use language that reflects our Christian values.
  • Faith Formation Behavior Policy

    If a child engages in a consistent pattern of misbehavior, parents of the child will be contacted and asked to discuss the issue with their child to encourage more appropriate behavior.

    If misbehavior persists, a parent or guardian will be required to attend classes with the child. Serious violations of the Code of Conduct may require the removal of the child from the classroom and continuation of the program in the home.

    The fees for the year 2026/2027 will be $130 per child, with a family cap of $375.00.
