I certify that the above information is accurate and complete to the best of my knowledge. I understand that the SJBHS has the right to reclaim the animal if any given information is found to be false. I authorize the release of veterinarian information related to my current and past pets. I understand that there are costs associated with owning a pet, that these costs can range from $400-$800 annually, and that there may be additional costs due to injury or illness.

I understand that this application does not guarantee the adoption of any pet and the requested pet will be adopted out to whomever is determined to be the best fit for the animal.  I understand that I may be denied adoption for certain animals and not others according to specific needs of the pet.  I understand that the pet I request may be adopted before my application is processed, that advertisement of said pet continues while my adoption is pending, and that the only way to guarantee a particular pet is "reserved" is by receiving a notifaction of approval from SJBHS adoption coordinator by phone or e-mail and placing a 48-hour hold fee for said animal.  Hold fees are non-refundable and expire after 48 hours if the animal is not picked up, unless special arrangements have been made with SJBHS adoption coordinator.

By signing this electronic application, I will comply with the adoption contract if I am approved to adopt from SJBHS.
