Consumer Consent to the Use of Electronic Transactions, Signatures and Records Disclosures and Notices

Your agreement with Monterey Financial Services, LLC will be in electronic form. However, a hard copy of the contract will be printed and stored with Monterey Financial Services, LLC, and Seller encourages you to print a copy of the contract and all related documentation for your records. 2) Receive electronic versions of certain records in connection with your transactions with Seller or its assignee. The records are those that may be used to provide or make available certain written consumer information to which you are legally entitled. 3) Electronically view communications related to your contract. For this purpose, you will receive an e-mail containing a link to a web site on which the communication will be posted. After you register your consent by clicking on "I accept", you will not receive from Seller a paper copy of the contract or any related disclosures, unless specifically requested.
Your Right to Withdraw Consent
If at any time you wish to withdraw your consent to conduct business electronically, you must follow the following steps: 1) If you have not completed the online process by confirming your contract, you may simply ignore the confirmation request email message that was sent to you. 2) If y ou has confirmed your contract by responding to the e-mail sent to you, you must contact Monterey Financial Services, LLC customer service at ( 800) 773-2804. How to Obtain Paper Copies. You may upon request obtain a paper copy of any electronic record which includes your consumer Information. You have two ways to do this: 1) You can print the record to your printer. 2) You can write to credit@montereyfinancial.com and request a paper copy of the electronic record. There may be a nominal f ee for the order of such documents. Hardwar e and Software RequirementsFollowing are certain hardware and software requirements for your computer system that must be met so you can enter into an electronic transaction, use electronic signatures and access and retain electronic records of consumer information. If you have any questions about this you should call Monterey Financial Services, LLC Customer Service prior to consenting. A Personal Computer, Tablet, Smartphone or another device able to connect to the internet with the ability to browse standard web pages. A printer needs to be available to the device should the user wish to print their own hard copy.

Terms of Application
To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. What this means for you: When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents. You hereby acknowledge that you are over the age of eighteen (18) years and that all of the information set forth in the credit statement is true, accurate and full and complete disclosure thereof. You are providing verbal consent to the client to submit your application under the Fair Credit Reporting Act and for the client and its partners with whom you are matched to obtain a consumer credit report from a contracted credit bureau. You understand that you are submitting an application for credit, and are consenting to the use of your credit report information. If this application results in a Retail Installment Contract or Closed-End Consumer Lease, you authorize any holder of the Retail Installment Contract or Closed-End Consumer Lease or any person, firm or corporation requested to extend credit thereunder, (including any employee or agent of any of them) to communicate with your employer in order to verify your employment. You authorize any holder of the Retail Installation Contract or Closed-End Consumer Lease, the creditor thereof, or any Attorney, debt collector or collection agency to communicating any and all information concerning this application or debt to any credit reporting agency or another creditor. By providing your email address, you consent to receive electronic information such as monthly billing reminders, statements, and collection notices. You also acknowledge that we and our partners may use all contact information provided to contact you regarding your application, Retail Installment Contract or Closed-End Consumer Lease, account status or future issues. You expressly consent and agree that we and our affiliates, agents, successors, assigns and service providers may use written, electronic and verbal means to contact you. This consent includes, but is not limited to, contact by manual calling methods, prerecorded or artificial voice messages, text messages, emails and/or automatic telephone dialing systems. You acknowledge that we and our affiliates, agents, successors, assigns and service providers may monitor or record all calls to and from their office. You agree that we and our affiliates, agents, successors, assigns and service providers may use any email address or telephone number you provide, now or in the future, including a number for a cellular phone or other wireless devices, regardless of whether you incur charges as a result. You further acknowledge and agree, that you will notify us and our affiliates, agents, successors, assigns and service providers in writing of any change in your name, address or employment within a responsible time thereafter. You agree that any and all acknowledgments provided by you in the Contract are provided to us in further consideration of the extension of credit to you and are essential terms of the Contract. Married Wisconsin Residents: No provision of any marital property agreement, a unilateral statement under Wis. Statute Section 766.59 or a court decree under Section 766.70 adversely affects the interest of the creditor unless the creditor, prior to the time the credit is granted, is furnished with a copy of the agreement, statement or decree or has actual knowledge of the adverse provision when the obligation to the creditor is incurred.
