    Note to training providers:

    1. Application for SISV (QS Division) CPD Programme should be submitted at least one month before the commencement of the event.
    2. Approval of CPD Applications will take at least three weeks from the date of application.
    3. The application course title and date should be the same as what you would inform your participants. To make any amendments, you can submit a re-application and request the secretariat to delete the earlier application.
    4. If your application has been successfully qualified for CPD, you are reminded to ensure that only the SISV (QS) attendance is recorded and uploaded in the Excel template provided within a month of commencement of course.
    5. Where a course is canceled or there is no SISV (QS) attendee etc., the attendance file is also required to email to qs.admin@sisv.org.sg.
    6. Processing of new applications may proceed only if the required attendance files are sent to us.
    7. Training Provider is responsible to collate and record important information on QS attendee full names as per NRIC/FIN nos. and membership registration nos. 
    8. An administrative fee of $216.00 (8% GST inclusive) is applicable for all CPD accreditation applications.

      CPD points awarded should be clearly stated in the proof of attendance given to participants via email communication.
    2. Ensure to submit the excel attendance file and signed attendance list within one month upon completion of the course. Download Attendance Template here.
