Form Tips:

Input the quantity for each product you need to order for each line item below. When a quantity is entered, the form will automatically check the box for that product. To proceed with your order, please be sure to leave the box checked. 

If you need to get a cost estimate, you can uncheck the box for each of the line items, and it will provide a total, but will not allow you to complete the order. 

Be sure all necessary products are checked before you submit the order. 

Tax:

All chapters based in Tennessee will be charged a 9.75% sales tax on products ordered. 

Upon submission your order, you have 90 days from the date the order was originally placed to inform ChapterSupplies@KappaDelta.org of any missing items or issues with the order.
