One Form Required Per Student (Payment will be required)
ALL students, new and returning, are required to complete this form.

We do have a limited number of pre-owned dresses / tux pieces to sell at a discount. If you are interested in this option for your dress, please indicate that in the question below. You will be charged the cost of a new dress now, and when sizing is done in the beginning of the school year, if we have attire in the appropriate size, you will be refunded $35 at that time.
