In consideration of being allowed to participate in any way in the Watertown Skating Club’s skating program / activities, the undersigned acknowledges, appreciates, and agrees that:
1. The risk of injury from the activities involved in this program is significant, including but not limited to the potential for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce the risk, the risk of serious injury does exist; and
2. I (My child) KNOWINGLY AND FREELY ASSUME ALL RISKS, both known and unknown, and assume full responsibility for my participation; and
3. I (My child) willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I (My child) observe any unusual significant hazard during my (My child’s) presence or participation, I (My child) will remove myself (him/her) from participation and bring such to the attention of the nearest official immediately: and
4. I (My child) for myself (him/her) and on behalf of my heirs, assigned personal representatives, and next of kin, hereby release and hold harmless the Watertown Skating Club, it’s Board of Directors, Pros and Instructors, and Taft School, with respect to any and all injury, disability, death or loss or damage to person or property.
This is to certify that I (As parent/guardian with legal responsibility for this participant) have read and do consent and agree to this release of liability and assumption of risk agreement and fully understand its terms. I (As parent or guardian) further understand that I have given up substantial rights by signing this form, and hereby sign it freely, voluntarily, and without inducement.
Photos of club activities may be posted on our website or Facebook. (If any names are used, it will only be first names.) I give permission for my child's photograph to be included on the WSC website or Facebook.
I uphold the USFS SafeSport Program and its policies and agree to abide by all codes of conduct.
