If you have are locked out of your home and need Tandem's assistance to gain entry, complete this form. Once you have submitted your information, submitted the form, and paid the fee, you can call the emergency line to coordinate entry. If you have not submitted the form and paid the fee, you will not be assisted.

YOU WILL BE ASKED TO SHOW ID BEFORE YOU ARE GIVEN A KEY OR ACCESS TO THE HOME. Anyone requesting assitance or entry other than the person submitting the form will not be given access or a key. You must be present and show your ID.
