Equinox Sports League Waiver & Player Conduct Form

In consideration of being allowed to participate in anyway, and at any time in a sporting event sponsored by Equinox, The undersigned:

1. Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own action, inactions or negligence but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.

2. Agree that prior to participating, they will inspect the facilities and equipment to be used, and if they believe anything is unsafe, will immediately advise their coach, supervisor, referee or League Coordinators of such condition(s) and refuse to participate.

3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.

4. Release, waive, discharge and covenant not to sue Equinox, their directors, commissioners, referees, employees, agents, facilities and sponsors from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of Equinox or its related "releasees" enumerated above or otherwise.

5. As a participant in the Equinox (DC) Leagues, I agree to act in a manner in which I respect my teammates, opposing players, coaches, and referees. As such, I will refrain at all times from engaging in the following:

a. Using profanity

b. Provoking and/or engaging in verbal, physical, and/or any other extra-curricular activities and/or altercations that detract or take away from the league game

c. Incite or cause others to engage in the activities laid out in “B” above and/or respond to, or escalate the actions of another participant or coach to such levels

d. Be disrespectful to any member, staff, or other employee, or guest of the club

e. Cause harm to any member, staff, or other guest of the club

f. Cause damage to any property in and/or around the club

g. Violating any policies set forth by the club governing the league

By signing below, I understand that the Club has taken great efforts to establish a safe and fun league for all participants.

I hereby acknowledge the aforementioned rules and regulations for player conduct and, agree that failure to comply with these rules as they are set forth above, I will be dismissed from the league, removed from the gym, and will forfeit any payment that I have made to participate in the league.
