Activity Registration Form
The activity registration form is required to request practice reservations and hold special events at RSS and Saluki Athletics facilities. Please complete this form for the following situations:
- Notifying the Competitive Sports Office that club related practices, home competitions, or special home events will occur at non-RSS and non-Saluki Athletic events.
- Requesting on-campus practice space(s) at RSS or Saluki Athletics facilities,
- Requesting reservations for special events located at RSS and Saluki Athletics facilities.
To report official trips, please complete the RSS Travel Request Form.

Event Staffing
Please consult with the Competitive Sports Office regarding sport officiating needs, ambulance services, and athletic trainer services. The Competitive Sports Office and/or your National Governing Body (NGB) may require that such services be provided.

Sport officials' rates will vary and will be set by the Student Officials' Association (SOA). RSS Sport officials will be provided after Competitive Sports programming needs have been met. When certified sport officials are needed, local scholastic assignors will set the officials' and terms for providing service.

The SIU Competitive Sports Office cannot guarantee that athletic trainers can be provided. Please be aware the Orthopeadic Clinic may cancel scheduled Athletic Trainer services with warning.

All RSS and SIU policies regarding food handling and catering MUST be followed. All servers must wear gloves. Individuals cannot handle money and food in the same gloved hand. Common allergens for all food must be clearly displayed on a sign. All items must be individually wrapped. Food vendors must appear on the SIUC Approved Caterers & Food Trucks list.
