Parent/Guardian Consent, Assumption of Risk & Release of Liability
I, the undersigned parent or legal guardian, voluntarily authorize my child to participate in soccer camps, recreational activities, athletic training, games, exercises, and other supervised activities organized or conducted by Memorial Indoor Sports Academy (MISA).
Assumption of Risk
I understand and acknowledge that participation in indoor soccer camps and recreational activities involves inherent risks, including but not limited to slips, falls, collisions, contact with other participants or equipment, sprains, fractures, concussions, serious bodily injuries, and, in rare circumstances, death.
I voluntarily assume, on behalf of myself and to the extent permitted by Texas law on behalf of my child, the inherent risks associated with participation in these activities, whether foreseeable or unforeseeable.
Express Release of Liability — Including Ordinary Negligence
To the fullest extent permitted by Texas law, I expressly release, waive, and discharge Memorial Indoor Sports Academy (MISA), its owners, directors, officers, employees, coaches, counselors, volunteers, agents, and representatives from claims arising out of my child's participation in camp activities, including claims arising from the ordinary negligence of MISA or any of the other released parties.
I understand and expressly agree that this release is intended to include claims arising from MISA's own ordinary negligence and the ordinary negligence of its owners, employees, coaches, counselors, volunteers, agents, and representatives, including negligent acts or omissions involving supervision, instruction, equipment, or facility conditions, to the fullest extent such claims may lawfully be released under Texas law.
This agreement does not release claims arising from gross negligence, reckless conduct, intentional misconduct, or any other liability that cannot legally be waived.
I acknowledge that signing this agreement may limit certain legal rights available to me. I understand that this agreement does not necessarily prevent my child from bringing claims that cannot legally be waived by a parent or guardian.
Emergency Medical Authorization
In the event of an injury or medical emergency, I authorize MISA personnel to contact emergency medical services and obtain reasonably necessary medical assistance for my child if I cannot be reached promptly.
I understand that I am financially responsible for medical expenses incurred on behalf of my child, subject to applicable law and any responsibility that cannot legally be excluded.
Parent/Guardian Acknowledgment and Consent
By signing below, I acknowledge that I have carefully read and understand this agreement, including the assumption of risk and express release of liability provisions.
I voluntarily consent to my child's participation in MISA's indoor soccer camp activities and agree to the terms of this document to the fullest extent permitted by Texas law.
