UWC Schedule Request Change Form

  • Emma will need at least three business days to change your schedule.  If you are unable to make your shift due to your schedule change, please fill out an absence form. 

    All shifts must be at least 2 hours. All shifts require a 30 minute break after 4 hours of consulting.
