I agree to perform certain services for Leuk’s Landing, Inc., a Michigan non-profit corporation. The Services include but are not limited to:
• providing direct animal care onsite at Leuk’s Landing facility
• petting and playing with the kitties onsite at Leuk’s Landing facility
• acting as a foster home for a Leuk’s Landing kitty (kitties)
• transporting kitties to the vet or a foster home
• photographing or videotaping on behalf of Leuk’s Landing
• cleaning onsite at Leuk’s Landing facility
• hosting groups at Leuk’s Landing facility
• assisting at special events
I further understand and acknowledge that certain risks--including but not limited to animal bites, scratches, contact with infectious disease and possible contact with strong chemical or cleaning agents, as well as risks from being on the premises-- may be associated with performing the Services.
In consideration of being permitted to perform the Services for the Leuk’s Landing, I voluntarily and knowingly waive any and all rights, claims or causes of action including, without limitation, those involving bodily injury or property damage to the myself, or to my family or property while the I am engaged, directly or indirectly, in performing the Services, whether or not caused by the negligence of Leuk’s Landing, its officers, directors, agents or employees.
In further consideration of being permitted to perform the Services for Leuk’s Landing, I agree to indemnify, defend and hold Leuk’s Landing, its officers, directors, agents and employees harmless from and against any and all liability, damage, loss, cost and expense incurred as a result of any claim, demand, or cause of action, brought against Leuk’s Landing, its officers, directors, agents or employees, jointly or individually, for bodily injury or property damage suffered as a result of my negligent, reckless or willful act or omission in the performance of (or failure to perform) the Services. In addition, while acting in the volunteer capacity, I understand I will receive no financial remuneration from Leuk’s Landing.
I represent that I am 18 years or older and that I understand the contents of this Waiver of Liability and Agreement to Indemnify. I further acknowledge that I have read and understand the contents of the Volunteer Guidelines which are incorporated by reference into this Waiver of Liability and Agreement to Indemnify.
This Waiver of Liability and Agreement to Indemnify shall continue in full force and effect until terminated in writing; and in the event of such termination shall remain applicable to all matters occurring or first arising on or before the date of such termination regardless of such termination.
