Reason for withdrawing classes *
I understand that once this document is signed and submitted to Hart Gymnastics Academy’s office, the drop request goes into effect immediately. If a class is dropped mid-month there will be no credits and/or refunds received for the remaining classes of the current month. If you wish to continue the current month’s classes and drop classes ONLY for upcoming month, please turn request form in by the 20th of current month.
