In accordance with Section 13 of the By-laws of the Park Slope Terrace Condominium (675 Sackett Street), the following is the Application for minor and major renovations, which must be completed prior to any alterations, additions, improvements or repair in or to a unit (excluding Sponsor).
Renovations that require board approval include but are not limited to: alterations that have structural implications (heating, plumbing, electrical, windows, walls, etc..), require permits to be drawn, expose the building to violations, or alter the exterior of the building. Alterations that do not require board approval include but are not limited to: paint, wallpaper, trivial repairs or other cosmetic work. If Owner is in doubt on whether an alteration requires Board approval, they should consult the Board or Management.
Minor Repairs & Improvements
Before commencing work, all contractors, regardless of scope of work, must provide management with required insurance documents and all contractors & sub-contractors must provide management with signed indemnification agreement.
Application Required Documents & Information
- Completed and signed Alteration Agreement (note that the deposit amount, if required, will be determined after submittal and prior to approval)
- Signed Indemnification Agreement by all contractors & sub-contractors
- Other Required information and documents from Unit Owner:
- Any and all drawings associated with the renovation
- Building department applications or permits (if required)
- Contact information of architect, contractor and engineer (if applicable)
- Written description of work to be performed

- Gather, complete and sign all the required documents listed above and scan documents
- Supply the requested information and pay any applicable fee (alteration agreement only)
- After receipt, Board and Management will review scope of work and plans and will determine the following:
- Whether the Board will engage the Building Architect or Engineer for consultation on reviewing plans
- How much, if any, security the Board will hold as per paragragh 1c of the Alteration Agreement
- Once Alteration Agreement is approved and security deposit is funded (if required), Management will send Unit Owner a fully executed Alteration Agreement
- Unit Owner/Contractor should coordinate start of renovations with Management in accordance with terms of Alteration Agreement
- All required insurance certificates must be on file with Management prior to the commencement of work.
