Buyers must submit this form before we ship hazardous material or allow the use of hazardous materials. A few examples of products this waiver covers: Sodium hydroxide, potassium hydroxide, both also known as caustic soda or casutic potash, lye or soap making lye. 

As the buyer of the hazardous materials I fully understand the material I am purchasing is considered hazardous and could cause bodily harm, or even death.

I assume all responsibility for its safe use and proper use, and storage and disposal.

I understand that all records of sales are available to law enforcement upon request.

I certify that I am at least 18 years of age.

I do hereby release Chemistry Connection, an Arkansas company, of all liability when transporting,  using and disposing of this product and further, indemnify, hold harmless and forever discharge Chemistry Connection and its agents, employees, officers, directors, affiliates, successors, managers and assigns, of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages and liabilities, of every kind and nature, whether known or unknown, in law or equity, that I ever had or may have, arising from or in any way related to my use of the products purchased.

By this Waiver, I assume any and all risk, and take full responsibility and waive any claims of personal injury, death or
damages, and agree not to sue Chemistry Connection, its owners, agents or suppliers or employees. 

THIS WAIVER AND RELEASE OF LIABILITY INCLUDES, WITHOUT LIMITATION, INJURIES WHICH MAY OCCUR AS A RESULT
OF (1) Buying the product(s), use of the product(s) (2) ANY uses of the product. (3)Disposal of the product.

The provision of this WAIVER AND RELEASE will continue in full force and effect even after the termination of the sale or use or the activities conducted with using the product(s)

I have read, understand and fully agree to the terms of this WAIVER AND RELEASE. I understand and confirm that by filling this electronic form out, signing it and then submitting this WAIVER AND RELEASE I have given up considerable future legal rights. I have signed this Agreement freely, voluntarily, under no duress or threat of duress, without inducement,promise or guarantee being communicated to me. My submission of my electronic signature on this form is proof of my intention to execute a complete and unconditional WAIVER AND RELEASE of all liability to the full extent of the law. I am 18 years of age or older and mentally competent to electronically sign and submit this waiver.
