By submitting this form, You, the participant, are aware that you are engaging in strenuous physical exercise and that the use of equipment, training and instruction, could cause injury to you. You are voluntarily participating in these activities and assume all risk of injury that might result. You agree to waive any claims or rights you may otherwise have to file a claim for damages against your 4K-Karate instructors or Lincoln Elementary School (District 156) for injury to you as a result of these activities. It is advisable and recommended to consult your physician before undertaking this or any exercise program.
