Before you start
Make sure you have the following information ready. This form doesn't have an autosave feature.
- Contact details (name, email and phone number) for:
- the person you're nominating
- 2 or more people making the nomination
- the organisation's executives if you are making a nomination on behalf of an organisation.
- Explanation of why your nominee deserves the award. You can either upload a file that you've prepared earlier (eg. word document or PDF) or type this out into the form.
- List of relevant jobs or roles your nominee has held.
- Letters of support from people who know about your nominee's service.

We collect personal information to provide Council services and perform our statutory functions. Your information will only be used and disclosed for the purpose of the administration of the Civic Honours Awards.
