Client Agreement

This Agreement for Performance of neuroBridge Therapy Services (“Agreement”) is made on {date} (“Effective Date”), between {child} ("Client"), and neuroBridge, LLC ("Company"), with address of P.O. Box 4362, Chattanooga, Tennessee, 37405.

RECITALS

Company is in the business of providing Occupational Therapy, Speech and Language Services, Educational Services, and Psychotherapy and other Mental Health Services (“Services”); and

Client wishes, and Company agrees to perform certain of these services for Client under the terms and conditions set forth in this Agreement;

NOW, THEREFORE, In consideration of the mutual promises set forth in this contract, it is agreed by and between the Company and Client as follows:

1. Agreement and Releases

This Agreement shall consist of this Client Agreement, terms and conditions listed on the website, the proposed therapy or other plan for the child (“Plan”), fee schedules, and any relevant waivers or releases. Each shall be incorporated by reference into this Agreement without further notation, and shall have the same force and effect as if stated fully herein. The documentation together shall be referred to as the Agreement.

2. Retention of Company.

• 2.1 Services. Company will perform Services for Client’s child (“Child”) as indicated on the Plan of Care.
• 2.2 How Services are Provided. The Services that the Company agrees to provide to the child shall be performed substantially in the following manner: 
  
  (A) Providing treatment for the Child at the school unless otherwise indicated in situations where school might be closed. This will be communicated to client in advance of treatment sessions. 2
  
  (B) neuroBridge, LLC will communicate with the school, including but not limited to administration, support staff, and teachers in direct contact with your Child regarding child’s treatment program and status unless otherwise requested by Client.
  
  (C) neuroBridge, LLC will determine the method, details, and means of performing the above-described services as best for your Child.
  
  
• 2.3 Disclaimer. Although neuroBridge, LLC provides various forms of treatment, therapy, and assessments, Occupational Therapists do not provide cognitive training.

3. Fees and Payment.

The Fees shall be indicated on the current fee schedule. Payment must be made in accordance with the Terms and Conditions on neuroBridge LLC.

4. Term.

This Agreement will be for the period as indicated in the Terms and Conditions or Plan of Care.

IN WITNESS WHEREOF, the parties hereto have caused this Client Agreement to be duly executed, sealed and delivered as of the date first above written.
