I am the parent or legal guardian of the Youth named below and I hereby grant the Near North Mobile Media Arts Lab Collective Corporation, and their partners in the Digital Creator North program, the absolute and irrevocable right and unrestricted permission in respect to any legal use of video and still imagery of my child or ward, including but not limited to the use, reuse, publishing and republishing of said imagery, in whole or in part, in any and all media now or hereafter known, for presentation, promotion, art, illustration, editorial, advertising, and publicity.

I understand that no royalty, fee or other compensation shall become payable to me by reason of such use. Furthermore, I hereby release and discharge the Near North Mobile Media Arts Lab Collective Corporation, and their partners in the Digital Creator North program, from any and all claims and demands arising out of or in connection with the use of images or likenesses, including without limitation any and all claims for libel or violation of any right of publicity or privacy.

This authorization and release is binding upon, and inures to the benefit of, the parties and their respective heirs, successors and assigns, including the legal representatives, licensees, and assigns of the Near North Mobile Media Arts Lab Collective Corporation, and their partners in the Digital Creator North program, as well as the person who signed below and who were photographed/videotaped.

As the parent or legal guardian of the Youth named below, I have full rights to sign this document on their behalf. This release shall be binding upon myself and my heirs, legal representatives, and assigns.
