This document will serve as the "signature on file" for all rental agreements in the name listed above and/or the name of the company. In the event that the undersigned (A) extends the Rental Period; (B) fails to return the Equipment at the expiration of the Rental Period; (C) purchases any additional services, supplies or accessories, the undersigned; (D) damage or loss of the rental equipment, I herby authorize Pretty Fancy, LLC to charge any and all costs associated with the rental of the equipment and/or the purchase of any equipment, supplies or accessories, including but not limited to, any and all hourly, daily, weekly or monthly charge(s), purchase price(s), finance charges on all unpaid balances this also includes applicable surcharges, damage costs incurred in retaking the equipment, repair costs and replacement costs, rental security hold amounts to the credit card listed above.

By signing this document, I agree and authorize the above terms of this credit card authorization form and in addition to the above authorization agreement, I authorize Pretty Fancy, LLC to authorize and validate the credit card above for the total of the order.
