Thank you for your interest in having your Juliette Girl Scout participate in Product Program(s). Please complete this form to sign them up and receive sales materials.

I give permission for my Girl Scout Juliette:

Fall Product Program Snack items and rewards for both programs will be available in your neighborhood Service Unit for pick up.

Girl Scout Juliette Participation Policies:

• All monies earned is the property of Girl Scouts of North East Ohio and is not to be retained by any one individual.

• I understand that product ordered may not be returned.

• I will ensure my Girl Scout Juliette follows appropriate safety guidelines.

• I agree to accept financial responsibility for all orders and monies for which my Girl Scout Juliette is accountable, and will observe and adhere to all procedures and deadlines imposed.

• Girl Scouts of North East Ohio will take necessary steps to recover all outstanding debts per our financial policies.

• I understand that if I have an outstanding balance/payments beyond set deadlines that it will impact my Girl Scout Juliette's ability to participate in future Product Programs.
