I give permission for my child/ren (listed above) to attend the Cineworld trip in November. I understand that the films (listed above) are morning shows and all children in Year 1 to Year 6 will walk back to school for lunchtime.

Please note that children and adults are not permitted to purchase or bring any food or drinks on the Cineworld trip.

FREE PARENT/CARER TICKETS - There may be some spare tickets.

If you would like to volunteer to join a class please complete the next section. Please note you will be walking to Cineworld with the class and may be responsible for a small group of children.

This is generally popular with parents/carers, if this is the case, we will put the names in a hat and draw them before half term, informing you as soon as possible after.

PARENT/CARER NO FOOD / DRINKS

If your name is pulled out of the hat please be aware we ask ALL children and adults to not given any of the children food or drinks. This is due to allergies and Natasha's law, and to ensure fairness, as not all children may have the same items, and we want everyone to have an equal experience.
