  • What is “Personal Information”?

    Personal information includes any factual or subjective information, recorded or not, about an identifiable individual.  This includes information in any form such as:

    -          Age, Name, ID numbers, Income, Assets, Household Composition, Residency Status, Rent Payment Record, Etc.

    -          Opinions, Evaluations, Comments, Social Status, or Disciplinary Actions, and

    -          Employment Files, Credit Records, Loan Records, Medical Records, Existence of a Dispute Between a Land=lord and a Tenant, Intentions (for example, to Acquire Goods or Services, or Change Jobs)

    Personal information does not include the name, title, business address or telephone number of an employee or an organization.

    Collection and Use of Your Personal Information

    Davis Drive Non-Profit Homes Corporation (hereinafter; “Davis Drive Non-Profit”) collects, retains and uses any personal information provided by you in this form for the following purposes.

    -          Considering your application for tenancy

    -          Verifying the information that you have provided in your application and its attachments relating to the administration and processing of your application for tenancy.

    -          Calculating your rent

    -          Meeting legal and regulator requireme3nts arising out of or relating to your tenancy.

    -          For the use of Davis Drive Non-Profit’s auditor to verify our financial records.

    -          For the purpose of contacting necessary services or your next-of-kin in case of an emergency.

    Disclosure of Your Personal Information

    Davis Drive Non-Profit may disclose the personal information provided by you to the following parties for the purposes described above.

    -          to any social agency providing any form of assistance to you, or other government subsidy under the Ontario Works Act, 1997 the Ontario Disability Support Program Act, 1997 or the Day Nurseries Act, or any government department responsible for social housing programs under the Social Housing Reform Act or Davis Drive Non-Profit’s housing portfolio operating agreements.

    -          To the Government of Canada, a department, ministry or agency of it, without further notice to you if the information is necessary for the purpose of administering or enforcing the Income Tax Act (Canada) or the Immigration Act

    -          To any agent working on behalf of Davis Drive Non-Profit for the purposes of complying with the Social Housing Reform Act

    -          To relevant agencies or next of kin in case of an emergency

    -          To credit bureaus and other businesses that provide credit or rental history information about you and

    -          To a third party in connection with the potential or actual sale, reorganization, merger, consolidation or disposition of the business of Davis Drive Non-Profit

  • Personal information contained on this form or in attachments is collected by York Region Social Assisted Housing Providers pursuant to the Freedom of Information and Protection of Privacy Act (R.S.O. 1990, C f 31) or the Municipal Freedom of Information and Protection of Privacy Act ( R.S.O. 1990 C , 56).  This information will be used to determine eligibility for housing applied for, continuation of housing and may be used for the appropriate rent geared-to-income charge.

     

    To Be Eligible For Housing

    -          You must be a permanent resident of Canada or have Refugee Claimant Status (copy of documents must be submitted)

    -          You must occupy or have need of a dwelling in the Province of Ontario:

    -          At least 1 person in your household must be 16 years old, or older.

    -          If you owe rent to any provincially assisted housing provider, you must have the provider’s agreement to waive or make payment toward the debt:

    -          You must be able to live independently with or without support services.

    -          For Seniors accommodations, at least one applicant must be 60 or over.

    I understand that if rental accomodation is provided to me it will be occupied by me and the persons listed on this application. 

    I understand that this application does not constitute an agreement on the part of the York Region Social Assisted Housing Providers to provide me with rental accomodation.

    I understand I must report any changes to family composition or household income.

    I understand that some housing provders may request additional information and complete a credit check.

To disclose the information given on this form to non-profit housing corporations/co-operatives, local housing authorities, the Ministry of Municipal Affairs and Housing and other municipal, provincial and federal departments and agencies that assist in the provision of afordable housing and social agencies providing social assistence to me and persons listed on this application.
