As the parent or legal guardian of the child named below, I hereby give my consent to allow my son, daughter, ward (hereafter “Child”) to tour the facility and/or perform volunteer services (hereafter "Services") for Leuk’s Landing Inc.
I fully understand and acknowledge that the Child's Services are to be performed subject to the Volunteer Guidelines of Leuk’s Landing Inc., a copy of which I have received and which I acknowledge reading and understanding; and that violations thereof shall be cause for immediate termination of the right to perform Services. All Services performed by my Child are strictly voluntary, without pay and compensation of any sort.
I specifically acknowledge that the Volunteer Guidelines require that a parent or guardian over the age of 21 be at the facility with, and supervising the Child the entire time that the Child is present. All such persons must sign an Adult Volunteer Waiver.
I further understand that public relations is an important part of volunteering at Leuk’s Landing Inc. On behalf of myself and my Child, I allow Leuk’s Landing Inc. to use any photographs taken of my Child for use in public relations efforts.
I further acknowledge that all Services are performed at my Child's own risk, as outlined below, and that I sign this document for my Child and the Child's heirs, personal representatives and administrators.
I further understand and acknowledge that certain risks--including but not limited to animal bites, scratches, contact with infectious disease and possible contact with strong chemical or cleaning agents, as well as risks from being on the premises-- may be associated with performing the Services.
In consideration of my Child being permitted to perform the Services for Leuk’s Landing, I voluntarily and knowingly waive any and all rights, claims or causes of action including, without limitation, those involving bodily injury or property damage to my Child, my Child’s family or property while engaged, directly or indirectly, in performing the Services, whether or not caused by the negligence of Leuk’s Landing, its officers, directors, agents or employees.
In further consideration of my Child being permitted to perform the Services for Leuk’s Landing, I agree to indemnify, defend and hold Leuk’s Landing, its officers, directors, agents and employees harmless from and against any and all liability, damage, loss, cost and expense incurred as a result of any claim, demand, or cause of action, brought against Leuk’s Landing, its officers, directors, agents or employees, jointly or individually, for bodily injury or property damage suffered as a result of my Child’s negligent, reckless or willful act or omission in the performance of (or failure to perform) the Services.
I represent that I am 18 years or older and that I understand the contents of this Parental Consent, Waiver of Liability and Agreement to Indemnify. I further acknowledge that I have read and understand the contents of the Volunteer Guidelines which are incorporated by reference into this Parental Consent, Waiver of Liability and Agreement to Indemnify.
This Waiver of Liability and Agreement to Indemnify shall continue in full force and effect until terminated in writing; and in the event of such termination shall remain applicable to all matters occurring or first arising on or before the date of such termination regardless of such termination.
