Adoption Terms & Conditions
I will provide ID tags for my dog within 48 hours of the date of this contract.

I understand that all dogs require an adjustment period to adapt to a new home and that it is generally unacceptable to judge a dog's long-term success in my home during this adjustment period. _

I understand that I must be diligent in creating an environment that will be successful for the dog, including creating a plan for reinforcing housebreaking, creating a secure environment for the dog (inside the house) when I am not home, and making note of any unusual behaviors so that they can be addressed by a trainer ifneeded.

I am adopting this dog to be a companion dog, not a guard dog. The dog will live inside my home, not outdoors.

The dog will not be allowed outdoors without proper supervision. When taking my dog outdoors, the dog will be on a secure harness or leash and will wear proper ID.

I will supervise the dog around adults, children, and other animals.
*
I will care for the dog in a humane manner and be a responsible guardian. Thisincludes supplying adequate food, water, shelter, attention, and exercise.

I will maintain current vaccinations and provide medical care as needed. I also agree to provide and administer monthly heart worm preventative.

I agree that any vet bills incurred after the dog is in my possession will be my sole esponsibility.
*
I agree to contact Scooter’s Pals before any decisions are made, by me or by my vet, to have the dog euthanized.

I agree to a follow-up home visit within six months to a year after adoption.

I am aware that dogs are different from people in their responses to human actions, that the actions of dogs are commonly unpredictable, and that Scooter’s Pals makes no representation as to the temperament or behavior of the dogs put up for adoption. I accept possession of this dog at my own risk and release Scooter’s Pals from liability for any damages to person or property that may be caused by this dog in the future.

I acknowledge that to the extent of their knowledge, Scooter’s Pals has fully informed me of any behavior issues this dog has, including but not limited to any history of biting. I accept the dog for adoption with this knowledge and I assume responsibility for the dog’s actions.

In the event that I move from the location noted on this Contract or if I have a change of phone number, I will notify Scooter’s Pals of my new address and phone number.

The dog is being adopted for myself and will not be sold, adopted, or given to another party except as indicated in the next clause immediately below.

If I can no longer keep the dog, the person(s) listed below have agreed to care for and/or adopt the dog. If it becomes necessary to return my dog to Scooter’s Pals, I accept full financial responsibilityfor any boarding fees or vet care costs incurred by Scooter’s Pals until a new home for my dog is found.

Adoption Fee
Our standard Adoption Fee of $250 helps us cover the costs of rescuing and fostering each animal we save – transportation, food, a wellness exam and essential vaccinations, plus any supplies an animal may need to prepare for family life. We welcome additional donations!
