By clicking the submit button, I agree to Hopkins Cat Rescue Inc. adoption process and will undergo an interview at the discretion of Hopkins Cat Rescue Inc.
By clicking the submit button, I understand that submitting an application does not guarantee approval for adoption, nor does it hold the animal I am applying for. It is entirely up to Hopkins Cat Rescue, in its sole discretion to approve me for adoption- and at any point may revoke approval.
By clicking the submit button, I understand Hopkins Cat Rescue Inc. will check my references including veterinary and personal.
By clicking the submit button, I understand there is an adoption donation associated with adoption of a pet from Hopkins Cat Rescue Inc. and that it is tax deductible according to IRS 501(c)3 guidelines. I understand this donation will ensure the organization is equipped to rescue another homeless pet.
By clicking the submit button, I agree NOT to sell, trade, giveaway or otherwise transfer ownership of my cat.
By clicking the submit button, I understand that if I no longer want my pet, or am no longer able to care for my adopted pet, I will be directed to surrender my pet to Hopkins Cat Rescue Inc. and provide transport to where Hopkins Cat Rescue Inc. deems appropriate.
By clicking the submit button, I agree to keep the animal as a companion and not allow the animal to be used for medical or other experimental purposes or for business purposes.
By clicking the submit button, if I have other animals at home, I attest to the fact that all of my animals are current on their annual vaccinations and are spayed or neutered if of age. I fully and completely release Hopkins Cat Rescue, its agents and volunteers from any claim, cause of action or liability for illness my other animals may develop, even if said illnesses may have been procured from the animal adopted from Hopkins Cat Rescue.

By clicking the submit button, I agree to provide my cat with necessary veterinary care including annual examinations; I agree to comply with the annual vaccination schedule for my cat as prescribed by my veterinarian.

By clicking the submit button, I agree NOT to use my car, or allow the use of my cat, for any activity prohibited by federal, state or local laws.
By clicking the submit button, I agree to permit an authorized representative of Hopkins Cat Rescue to visit my residence any time after adoption to examine my cat an ensure I comply with the terms of this agreement.
By clicking the submit button, I understand that the right to possession and ownership is not absolute, but conditional only and is subject to termination upon demand if at any time Hopkins Cat Rescue determines, in its sole discretion, that the conditions specified in this agreement have not been fully complied with, or if it is determined by Hopkins Cat Rescue, in its sole discretion, that the animal belongs to some other person. In either event, I agree to return the cat to Hopkins Cat Rescue upon demand.
By clicking the submit button, I understand there is no "cooling off" period, and that if I no longer want or can no longer care for my adopted pet, I agree to notify Hopkins Cat Rescue Inc. BY EMAIL and PHONE and provide a 14 day period to allow Hopkins Cat Rescue Inc. to make arrangents for my pet to be taken back into rescue.
By clicking the submit button, I agree to indemnify and hold harmless Hopkins Cat Rescue Inc. against any losses, lawsuits, claims, injury, damages incurred by me or to any persons or property by my adopted pet, once adoption has been completed.
By clicking the submit button, I understand that Hopkins Cat Rescue Inc. will disclose any of the pet's health or behavior issues known by the above named rescue group before adoption is completed, but Hopkins Cat Rescue cannot guarantee the health or temperament of the animals adopted from it. Hopkins Cat Rescue, its past, present and future officers, directors, agents and employees shall not be responsible for any conditions or illness that the animal may have or may develope or for any damage or injury to any person or property that the cat may cause.
