Consent Form
Dear Parents/Guardians:
Project Alpha is a workshop develop by Alpha Phi Alpha Fraternity, Inc. As a Chapter of Alpha Phi Alpha Fraternity, Inc., Delta Beta Lambda Hampton, VA Chapter conducts these workshops in Tidewater area which provides an environment where young males receive constructive information from adult males about sexuality, manhood and relationship issues. Participants also learn problem-solving skills and have an opportunity to share their own experiences and ask questions in an all-male setting.
The Brothers/Chapter Members of Delta Beta Lambda Hampton, VA, Chapter of Alpha Phi Alpha Fraternity, Inc., will provide participants information on HIV/AIDS, other sexually transmitted diseases, teen pregnancy, abstinence/virginity and condom use. Due to the sensitive nature of the workshop content, all participants will need the consent of a parent or guardian in order to attend.
No one will be admitted to the workshop without a signed consent form.
My signature below grants permission for my son to attend the Project Alpha Workshop, which is sponsored by Alpha Phi Alpha Fraternity, Inc. and the March of Dimes and conducted by Delta Beta Lambda Hampton, VA Chapter of Alpha Phi Alpha Fraternity, Inc. I fully understand that during the workshop, participants will be exposed to, and may discuss, information and issues related to sexuality and relationships, HIV/AIDS, and other sexually-transmitted diseases, teen pregnancy, abstinence/virginity and condom use.
