  • I, as Parent or Guardian, give permission for:

  • FIELD TRIP: High School Leadership Conference
    TRIP DATE: November 19-21, 2023
  • Mode of Transportation: parent/teacher-driven vehicle

    1. Students will leave ACS campus: 11:00 pm on 11/19
    2. Students will return to ACS campus: 4:00 pm on 11/21
    3. Administrator in charge: Mrs. McHale
    4. Dress Code: Casual/Modest
    5. Cost: 335.00 (4 to a room) or $415.00 (2 to a room) for the conference, transportation and parking, and hotel stay, t-shirt, tour and boxed lunch on tour.
    6. Students need to bring money for the purchase of four meals

    Permission Statement

    I, the undersigned, being the parent or legal guardian of the student named above, do hereby consent to the participation of my student in the above stated activity. Further, I certify that my student is physically fit and able to participate in this activity.

    I, the undersigned, being the parent or legal guardian of the student named above, release Atlantic Christian School, located at 391 Zion Road, Egg Harbor Township, New Jersey, its agents, assigns, employees, and volunteers from any liability whatsoever arising out of injury, sickness, or damage which may be sustained by said student during the course of said activity.
